The following requirements must be provided prior to the First Day of Care:
- Current immunization records. These records should be updated annually, and as new immunizations are administered
- Health Appraisal. These records should be updated annually for children ages 3 and above. For children under the age of 3, the health appraisal must be updated at each well child check
- Child Well visit summary. Every time the child visits the doctor’s office
- DHHS child in care statement
- State of Michigan Child information record cards
- ACD food program enrollment forms
- Mi Casa es su Casa completed contract
- Mi Casa es su Casa child questionnaire
- Ages & stages questionnaires
- First week of tuition
- Three weeks deposit of tuition to cover 3 weeks notice given at end of care
